Award Nomination Form

The deadline for submitting nominations is November 1. There are two exceptions to this deadline: The nomination deadline for the Distinguished Research Lectureship is June 30, and the deadline for the Ann M. Voda American Indian/Alaska Native/First Nation Conference Award and the conference awards is February 1. Please check the respective award sites for the submission guidelines. To nominate an individual for more than one award, you must submit a separate form for each award.
